This allows us to deliver coordinated, high-quality care tailored to your individual needs as our client. You may also use your own allied health professional as long as they meet our requirements.
*If you rely solely on Government funding, the services you have access to will be determined by your funding stream and level of care. However, if you would like more services than Government funding can provide, you can add those to your care plan at your own cost.

Tile: IHP – Interim Hospital Package

If you have a recent visit to hospital but need care to help transition you to home living, an IHP may be supplied via the Hospital. This can be up to 12 weeks of care as you settle and recover back at home. Hospitals do allocate which provider you use, however you can choose which one you would like to go with.

This is a short-term program where up to 16 weeks of home care can be provided using additional funding to those currently with CHSP and SAH. It is to provide additional aid when recovering from illness, injury or a recent hospital stay.

Palliative Care
Palliative care is specialised support for people living with a life‑limiting illness, and its purpose is to improve comfort, dignity, and quality of life. We can offer you support to maintain your comfort at home. Our extensive experience in delivering palliative care to individuals with diverse needs and personal challenges makes our team well-equipped to provide compassionate, tailored support during this important stage of life. These services complement any Palliative Care provided by Silver Chain.
End of Life Care
The End of Life Pathway provides a higher level of funding for people who have been diagnosed with 3 months or less to live. The pathway offers additional funding for services such as nursing care, personal assistance, and emotional support, tailored to the individual’s needs. Eligibility is determined on a high-priority basis by an aged care assessor and importantly being a Support at Home participant is not a pre-requisite.
