Support at Home can offer a wide range of services that you can access with government funding, depending on your funding level, which services have been allocated to you following your assessment and are outlined in your care plan.

Short Term Restorative Care
The Restorative Care Pathway can be accessed separately or in an addition to a participant’s ongoing Support at Home budget. This pathway can offer up to 16 weeks of restorative care and with an additional budget of between $6000 and $12,000 if required.
It's intended to help you regain your independence with daily tasks and focuses mainly on allied health services to help you regain your strength, especially following discharge from a hospital.
Eligibility is assessed via an aged care assessment which can be and can be conducted by an assessor whilst the participant is in the hospital.
You may also be able to have access to assistive technology and home modifications through the Assistive Technology and Home Modifications (AT-HM) scheme if needed.
