Support at Home is the government’s new program which has replaced Home Care Packages to provide the support you need to stay safe and independent in your own home. It brings with it a number changes, including funding levels, access to services, products, equipment and home modifications, with two new programs added – a Restorative Care Pathway to help you maintain or recover mobility and dedicated funding for an End of Life Pathway with specialised care.
However, we know that these changes can be confusing, so we're only a phone call away and can help you with any questions about how these changes will affect you. CPE Home & Community Care is approved to provide care for all levels, services and programs within Support at Home.

Classifications of Clients
When you receive Support at Home funding you will fall into one of three categories depending on the date that you have been approved for funding. This determines the conditions that will apply to you when your funding becomes available, and your services begin. The main differences between these three categories, concern co contributions to service fees and which services are available.
Grandfathered, Transitional & Direct Entry
Grandfathered
Approval for funding before or on 12th September 2024
If you did not pay a contribution prior to Support at Home, you will not have to do so now.
If you did pay a contribution you will continue to pay but not more than you paid before.
You will be able to continue using your funding for services that have been selected by you and your care manager, provided they are on the list of approved Support at Home services.
Transitional
Approval for funding after 12th September 2024 but before 1st November 2025
Following the 1st of November, you will be assessed in accordance with the Age Pension Test to determine your contribution rate.
You will be able to continue using your funding for services that have been selected by you and your care manager, provided they are on the list of approved Support at Home services.
Direct Access
Approval for funding on 1st November 2025 or later
Prior to your funding being released you would be assessed in accordance with the Age Pension Test to determine your level of co contribution.
You will be able to use your funds only for the services which have been approved for you on your Notice of Determination letter. However, if you would like to have other services pai for out of Support at home you can apply for a review.
Contributions
The percentage you will be asked to contribute towards your care is based on the Income test for Grandfathered clients, the Age Pension Assessment for Transitional and Direct Entry clients and also on the classification of the service you are accessing.
If you are finding it difficult to pay these contributions you can apply for assistance and we can help you with this
